What do you need to submit a claim?
Lost or Stolen Device
Report the loss or theft of your device at the nearest police station and obtain a South African Police Service (SAPS) case number.
Complete an affidavit that includes: Your full name and surname, ID or Passport number, Contract Cellphone number, IMEI number of the device, ITC number and SAPS case or OB number and a brief description and date of the incident.
